Surrogate Quick Questionnaire

To become a surrogate mother, you must meet the following requirements:

• Have had one or more complication-free pregnancies and deliveries
• Be between 21 and 37 years old
• Be a U.S. citizen or a Green card holder
• Be healthy and have a BMI less than 30 (Check your BMI)
• Not use illegal drugs or tobacco, and not alcohol dependent
• Never been convicted of a felony Have access to reliable transportation
